To reserve your appointment, we kindly ask that a credit card be kept on file. You will not be charged until the day of your appointment.

We offer a phone consultation for individuals who would like to learn more about our services and treatment approach before scheduling an initial appointment. This call is designed to answer general questions and mutually determine whether our practice is a good fit for your needs. Please note that the consultation does not include specific medical advice and does not establish a doctor–patient relationship. If you choose to move forward, the cost of this consultation will be applied toward your first appointment.

After submitting the appointment request below, you will receive an email with a form that must be completed. Note: This email may go to your ‘Junk Folder.’ If you do not receive it, please contact our clinic so that we may assist you.

Once we receive and review your form, you’ll receive an email confirming your appointment time. Please note that we are unable to conduct appointments before this step is completed.
