If you would like to become a new patient, we require a 10 minute phone consultation prior to establishing care which may be scheduled below. The purpose of this call is to answer general questions about the services we offer, our treatment approach, and to determine if this clinic may be able to provide you with the appropriate care for your unique situation. Please note that this initial phone call is not utilized to provide any medical services or advice and does not constitute a doctor-patient relationship. 

The cost of the initial phone consultation is $50, which will be applied to the cost of your first visit. If for any reason our clinic determines that we are unable to see you as a new patient, the cost of the phone consultation will be waived.
